To organise a prescription you can email us at kmicb.summerhillscripts@nhs.net or hand us your prescription in person.

If sending an email for anything other than prescriptions, please contact us at kmicb.summerhilladmin@nhs.net 

We are unable to book appointments over email. Please call us or use the Econsult service.

 

APPOINTMENTS

Summerhill operates a total triaging service, E-consult. This enables us to ensure that urgent matters are dealt with appropriately and all non-urgent are seen at the appropriate time.

Please complete an online E-Consult form via our home page using the contact your doctor on line pop up link. If you do not have access to the internet our staff will assist you over the phone to gain access to the triage system.  Our E-consult service is available between 8am and 2pm weekdays (excluding public holidays)

The majority of medical problems can be dealt with by telephone and therefore, face to face appointment will be given if it is not possible to manage your condition via telephone.

As more consultations are being dealt with by telephone all calls made by the doctors and staff to patients will be recorded, for training and monitoring purposes.  All call recordings can only be accessed by the Practice Manager or the Reception Manager.  No other staff have access to ensure information is kept secure and confidential.

If you do not wish you have your call recorded, please advise the staff at the beginning of the consultation.

Some nurse appointments are also now being done by telephone.  You can ring reception at any time to request a nurse appointment.

Please contact us if you are not able to attend the appointment so we can rearrange a different date for you, this frees up appointments for people who can make it. We really appreciate your help with this.

In order to provide the right level of care, we are required to hold personal information about you on our computer systems and in paper records to help us to look after your health needs, and your doctor is responsible for their accuracy and safe-keeping. Please help to keep your record up to date by informing us of any changes to your circumstances.

Confidentiality and Personal Information

Doctors and staff in the practice have access to your medical records to enable them to do their jobs. From time to time information may be shared with others involved in your care if it is necessary. Anyone with access to your record is properly trained in confidentiality issues and is governed by both legal and contractual duty to keep your details private.

All information about you is held securely and appropriate safeguards are in place to prevent accidental loss.

In some circumstances we may be required by law to release your details to statutory or other official bodies, for example if a court order is presented, or in the case of public health issues. In other circumstance you may be required to give written consent before information is released – such as for medical reports for insurance, solicitors etc.

To ensure your privacy, we will not disclose information over the telephone or fax unless we are sure that we are talking to you. Information will not be disclosed to family, friends or spouses unless we have prior written consent, and we do not, leave messages with others.

You have a right to see your records if you wish. Please ask at reception if you would like further details about our patient information leaflet. An appointment may be required. In some circumstances a fee may be payable.
